A consent form people can sign on their phone

A consent form records that someone was told something and agreed to it. This page holds the wording you supply, tick-box statements, a typed name and a drawn signature, with optional date of birth and emergency contact. It's a way to collect a form, and doesn't decide whether your wording or process is enough where you are.

When to use it

  • A massage therapist or salon wants a form signed before a first session.
  • A club needs a parent's permission for a trip.
  • A photographer wants agreement before using pictures on social media.
  • A class provider asks people to confirm they've read the safety information.

What a good one includes

  • A plain description of what's being agreed to, in words the person will really understand.
  • Separate tick boxes for separate things. Someone can accept treatment without accepting marketing texts.
  • Which statements are required and which are optional, as the form marks them.
  • What you'll do with their details and how long you'll keep them, if that's accurate for you.
  • An invitation to ask questions before signing, since consent given in a rush is worth less.
  • Who is signing and for whom, particularly when it's a parent or guardian.

Make one by saying what you need

Text it, type it or say it, the way you'd tell a colleague:

Consent form for Lumen Skin Studio before a first facial: skin conditions and medication declared, can stop any time, emergency contact, date of birth.

Worth knowing. Have your wording checked by your insurer, regulator or a lawyer before you rely on it, because the rules for health, minors and photo use differ widely, and this template doesn't supply legal wording. If you handle health details, check your privacy duties first, such as HIPAA in the US or data protection law elsewhere. Our terms leave that responsibility with you.

Questions

Is a signature on a phone legally valid?

Electronic signatures are accepted in many countries for many purposes, but not all. Some medical, legal or regulated settings have stricter rules, so check yours.

Can a parent sign for a child?

Often, but who can consent and for what depends on the child's age, the activity and local law. Confirm before relying on it.

Can someone withdraw consent?

In many settings, yes. Say how to do that on the form, and act on it.
